ATC Digital Data Submission Information (DDSI) Form
The ATCTM Digital Data Submission Information Form (linked below) is to be
used for all digital data submissions to the ATC.
Please make sure your data have been uploaded to the ATC before submitting the DDSI form.

Please note that as of March 1, 2014,
imaging and treatment planning
data submissions for all NRG Oncology clinical trials are to
be submitted using TRIAD. Please use the
IROC Digital Data Submission Information Form
(see instructions)
for all data submissions to TRIAD.
Please do not use the ATC DDSI for this purpose.
