RTOG Breast 0319 ITC Submission Checklist  

DATA SUBMISSION CHECKLIST
(All initial data is due within one week of the initiation of treatments)

	Digital patient data (CT's and contours)
	Digital planning data (Beams, doses, and DVHs)
	Film copies of simulator films, first day portal radiographs and first first set of orthogonal, isocenter verification, sim and port films (films may be substituted with digital images in either RTOG or DICOM 3.0 format, contact ITC if you have questions)
	Hard copy isodoses, axial, sagittal and coronal through isocenter for total dose plan in absolute dose (Gy or cGy)
	T2 form (available from ITC web site http://itc.wustl.edu/)
	email to itc@wustl.edu AFTER all digital data has been transfered
