Image Fusion Information (within 1 week of start of RT)

• IM Form - PET information
• PET/CT Data as DICOM-formatted digital data*
• CT Data, if different from planning CT*
• Color images of fused data for PET and CT scans. (These should be submitted as color JPEG images)

Initial Dosimetry Information (within 1 week of start of RT)

• For CT-only plan*
• Digital Data Submission Information Form (DDSI)
• CT Data
• Structures
• Digital Dose Data and Beam Geometry Data
• Digital DVH Data

• For PET/CT plan*
• Digital Data Submission Information Form (DDSI)
• CT Data
• Structures
• Digital Dose Data and Beam Geometry Data
• Digital DVH Data

Final Dosimetry Information (within 1 week of RT end)

• End of Treatment Form (T1)
• Copy of the daily Dose record (T5)
• Color Isodoses (T6) submitted as either JPEG images or color hardcopy

*  Please note: For each patient, please submit the following data sets in separate directories on the ITC SFTP server or CD media:

1. PET/CT images as DICOM files

2. CT-only plan (CT, Structures, Plan, Dose) as DICOM or RTOG files

3. PET/CT plan (CT, Structures, Plan, Dose) as DICOM or RTOG files
