Facility Questionnaires for RTOG Protocols supported by the ATC
A Facility Questionnaire is to be submitted by institutions seeking credentialing to participate in clinical trials protocols that involve data submission to the Image Guided Therapy QA Center (ITC). For each protocol, the facility questionnaire provides information about the personnel, resources, and clinical process used to treat patients on that protocol. The current version of the Facility Questionnaire consists of three parts. One or more of these parts must be completed for each protocol, depending on the treatment techniques to be used:- Part I - General information for 3D-CRT and IMRT
- Part II - IGRT systems and process
- Part III - Heterogeneity Corrections and Motion Management
Facility Questionnaire forms are available as fillable Word documents from the RPC website.
Completed forms should be submitted to the RTOG:
